Erie's One-on-One Health Education
Through one-on-one education, patients meet with a trained health educator to discuss nutrition, health habits, barriers and establish self-management goals. Patients can receive one-on-one education for:
- Diabetes
- Prediabetes
- Gestational Diabetes
- Hypertension Management
- General Health and Wellness
- Asthma
- COPD
- Smoking Cessation

This is a 26-week program of 1-hour sessions. It is for people who are pre-diabetic. We focus on ways to lower your chances of getting diabetes–using nutrition choices, practicing physical activities, managing stress, and other healthy habits. Participants must prequalify to participate. Open to established Erie patients.
This 8-week program meets 1 time per week. It is for patients with uncontrolled blood sugar levels (diabetes). It is a chance to learn about the importance of nutrition, physical activity, medication, and medical follow-up to manage your condition and help prevent more severe health issues. Open to established Erie patients.
This drop-in support group meets 1 time each month. It is for people with pre-diabetes or diabetes. Each month we discuss different diabetes topics like Knowing Your Kidneys, Preventative Care, and Making the Most of Doctor Visits. Open to the community.
This drop-in support group meets 1 time every other month. It is for people with pre-diabetes or at risk of pre-diabetes to learn how to prevent or delay diabetes with lifestyle changes. Open to the community.
This drop-in group meets 1 time every other month. We share easy, affordable, and family-friendly recipes. We teach individuals with limited food budgets how to shop for and cook healthy meals. Open to the community.

This drop-in group meets
1 time every other month. It is for parents and caregivers of children aged 6-13 to discuss and learn about supporting their family's health with nutrition, physical activity, sleep, and stress management. Open to the community.
Started by an Erie doctor who wanted to teach her patients how to fit exercise into their daily routine, Erie Running Club meets regularly from July to October and is open to individuals or families, Erie patients and community members. We teach you how to gradually become stronger and prepare you to run or walk in a 5K race. No running experience is needed. Open to the community.
This drop-in group meets 1 time every other month. It is for people interested in learning how to read nutrition labels, what is the My Plate Method, and how nutrition can impact your test results for cholesterol, blood sugar levels, and blood pressure. Open to the community.
