How do I get into my Erie MyChart Account?

1. Sign in to google play or apple app store
2. Type in the search bar “MyChart”
3. Press the “Download app” button
4. Once the app is downloaded, press the “Open” button
5. In the MyChart app search bar type in “Erie”
6. Select “Erie” as your organization

To sign up for Erie MyChart, follow these 2 steps:

1. Get an activation code. Just ask us.

  • Call Erie at 312-666-3494 during normal business hours, and choose the MyChart option in the phone options. OR if you are at Erie, ask the Front Desk staff to help you sign up for Erie MyChart.
  • Erie staff will send a TEXT or EMAIL with a link and an activation code to sign up for Erie MyChart. The text or email link expires in 24 hours. The activation code expires in 45 days, OR at the moment you request a new one.

Tap or click the link. It will take you to myeriehealth.com.

You might have an activation code on your After Visit Summary. If you do, then in a web browser you can go to myeriehealth.com and follow the steps below.

2. Use the activation code to create your account.

  • Click the link in the text or email that Erie gave you.
  • OR go to myeriehealth.com in a web browser on your smartphone, tablet or computer. Once there, tap the green Sign up now button.

If you clicked the link from Erie, your Activation Code will already be in the box.

  1. If the code is not there, enter it into the activation code boxes.
  2. Enter your ZIP Code, and Date of birth.
  3. Click Next.
  1. Create a Username and Password following the instructions on the screen.
  2. Click Next.
  1. If you want to get email or text notifications when new information is available (like test results or messages from Erie), click Yes to turn it on, and enter your email address and cell phone number.
  2. Click Sign in. You will be taken to your personal Erie MyChart home screen.

If you complete these steps and still have trouble signing in, call Erie during normal business hours at 312-666-3494. Or if you are at Erie, ask to speak with a Care Coordinator.

To sign in to Erie MyChart From a web browser.

  1. After you have signed up for Erie MyChart, to sign in, go to myeriehealth.com.
  2. Enter your Username and Password.
  3. Click Sign in.
  4. When you click Sign in you may have 1 more step to sign in to your account. This step helps protect your health information. After you click Sign In:
    • MyChart may send a code to the email address or cell phone number in your MyChart account - you choose which one. (The code can only be sent to phone numbers that can receive text messages, or to email addresses, and cannot be sent to home phone numbers. The code is valid for 20 minutes from when you request it).
    • When you get the code, you type it into the box that appears on your MyChart sign in page.
  5. Then, you will be taken to your personal Erie MyChart Welcome Screen.
    If you have not signed up for Erie MyChart yet, you need an activation code from Erie to do so. Call Erie at 312-666-3494. Or if you are at Erie, ask Erie Front Desk staff to sign up.

To reset your password in Erie MyChart:

  1. At myeriehealth.com, tap or click Forgot password?
  2. Enter your username, zip code, and date of birth.
    If you do not remember this information, call Erie during normal business hours at 312-666-3494. Or if you are at Erie ask to speak with a Care Coordinator.
  3. Click Next. A page will appear asking you to verify your identity.

Next, verify your Identity.

  1. If you have a valid email address on file in Erie MyChart, look for an email from MyChart with a code to verify your identity.
  2. With the code from the email, go back to the Erie MyChart window to verify your identity. Enter the CODE.
  3. Tap or click verify. You will then be taken to your personal Erie MyChart Welcome Screen.

If you still have trouble with your password after doing these steps, or do not have a valid email address on file in your Erie MyChart account, call Erie during normal business hours at 312-666-3494, and choose the MyChart option in the phone options. Or if you are at Erie ask to speak with a Care Coordinator.

To change the language of Erie MyChart, in the MyChart App or at myeriehealth.com, click or tap on the small globe icon. It may be near the top OR the bottom of the page.

Then, look for your language in the drop-down menu and click or tap on it.

Your Erie MyChart account is protected with 2-Step Sign-In.

  • When you sign in to Erie MyChart, MyChart will send a code to the email address OR cell phone number in your MyChart account - you choose which one.
  • When you get the code, you type it into the box that appears on your MyChart sign in page.
  • Please note: For sending the code to phone numbers, the process does not work with home telephone numbers. It only works with a phone number that receives text messages, or by sending the code to your email address. The code is valid for 20 minutes. Please make sure that you have opted in to receiving emails and text messaged from Erie MyChart.

Share Blood Pressure Numbers with My Provider

Sharing Your Blood Pressure Reading in Erie MyChart

When your provider assigns it to you, you can share your home blood pressure readings with your Erie Care Team. This helps track your health over time. The steps are the same using a smart phone, tablet or computer, in the MyChart App or in a web browser. It will just look a little different.

In the MyChart App, log in to your Erie MyChart account.

  1. Scroll down in your home screen feed and find VIEW TASKS.  Then click on VIEW TASKS. (If you don't see the prompt in your home screen feed, search in the menu for TO DO.)
  2. In your TO DO, click on “Check and record your blood pressure and heart rate.” (This task will be visible after a provider assigns it to you.)
  3. Enter your Pulse, Blood Pressure and Weight.
  4. Press DONE.
  5. Then press the BACK (<) Icon at the bottom of the screen.
  6. A Pop-Up will ask you if you want to save your changes. Click SAVE. (If your readings are outside of normal ranges, call your provider to talk about the results.)
  7. Click CONTINUE SAVING. You can now log out of the App.

If you need help with MyChart please ask to talk to an Erie care coordinator. Make an appointment with your provider to review your home blood pressure readings.

Sharing my record or getting access to a family member's Erie MyChart record

You might need access to a child or parent's health record. This is called proxy access. You would use this to help manage the health of your child from 0-11 years old, or of an adult when you are in charge of managing their health.

At Erie, 12 - 17 year olds have their own Erie MyChart accounts. We encourage adolescents to have increased ownership of their health information as a part of preparing for the transition of fully managing their own care in adulthood. Click here to learn more about adolescent care at Erie.

Identification:

  • Adult Patients – 18 Years and Older, bring your picture ID (driver’s license, State ID, passport).
  • Patients Under 18 Years, if accompanied by an adult other than the parent or legal guardian, the patient must bring a picture ID and consent form completed and signed by the parent or legal guardian.

Information about your symptoms and medications you or the child patient is taking:

  • Bring a list of questions and/or symptoms
  • Bring a list of medications you or the patient are currently taking, including the strength and dosage,
    • or just bring the bottles with you

  • Bring a list of over the counter medicines, vitamins, supplements and/or home remedies you are using
  • Bring a list of known drug allergies
  • Bring the name, address and telephone number of the pharmacy you prefer

Log in to Erie MyChart a few days before your visit to fill out forms or questionnaires assigned to you, and to update your personal information and insurance ahead of time.

If you are enrolled in Erie MyChart, you can log into your account up to a week before your visit to complete the PreCheck-In process.

You may get a text, email, or notification through the MyChart app, reminding you to do your PreCheck-in. Click on those links to complete the PreCheck-In process.

If you are not enrolled in Erie MyChart, and we have your email address, you may get an email a few days before your appointment with instructions on how to log in.

Front desk staff can also help you enroll in Erie MyChart. In MyChart you can fill out forms or questionnaires assigned to you, update your insurance, make payments, see your lab results, message your provider and more!

Get Ready for Your In-Person Visit
  • Save time with PreCheck-in, by logging in to Erie MyChart a few days before your visit, to fill out forms or questionnaires assigned to you, and to update your personal information and insurance before your visit.
  • Please wear a mask when entering any Erie health center.
  • If you are a new patient, please arrive 30 minutes before your appointment to register.
  • If you are an established patient, please arrive 15 minutes before your appointment.
  • Let the Front Desk team know you have arrived, once you enter the health center.
  • Be sure to ask questions if you don’t understand something. Be honest with your provider—even about sensitive subjects. Write down instructions given to you by your provider.
Get Ready for Your Video Telehealth Visit (for established Erie patients only)

Visit eriefamilyhealth.org/telehealth to prepare for your telehealth video visit. Please remember, for pediatric telehealth video visits the child must be present.

Save time with PreCheck-in, by logging in to Erie MyChart a few days before your visit, to fill out forms or questionnaires assigned to you, and to update your personal information and insurance before your visit.

If You Need to Cancel

If you are unable to keep your appointment, please call us at 312-666-3494 or 847-666-3494 at least 24 hours before your visit so that your appointment slot can be given to another patient.

You can also use myeriehealth.com to alert us of your need to cancel.

In an Emergency

If you or a loved one are having a medical emergency, call 911 or go to the nearest hospital Emergency Room.

Urgent Care
If you are an Erie patient and are not having an emergency, but are sick at a time when we are not open, call 312-666-3494 or 847-666-3494. We have providers on call 24/7/365.

Same day urgent care appointments
All of our sites reserve some appointment times for unscheduled urgent care for our patients. Call 312-666-3494 or 847-666-3494 to ask about a same day urgent care appointment.

Do not just go to a site; you must have an appointment.
