Erie Helping Hands and Erie Humboldt Park health centers will be open from 8:30 a.m. to 12:30 p.m. on Monday, May 31 for urgent care and telehealth appointments.

Erie Family Health Centers is partnering with Northwestern Medicine and the City of Chicago through its Protect Chicago Plus initiative to bring the COVID-19 vaccine to residents of the Humboldt Park and Garfield Park neighborhoods. (Read more about Protect Chicago Plus here.) You need an appointment to receive your vaccine.
The vaccine is the best tool we have to keep people from getting sick and dying from COVID-19, safely spend time with our loved ones, and end this pandemic.
These things are true about all of the COVID-19 vaccines being used in the United States:
- The COVID-19 vaccines are completely free and available to everyone- with or without insurance.
- You don't have to be a U.S. citizen to get a COVID-19 vaccine.
- They have been properly tested on thousands of people of all genders, races and ethnicities and have proven to be very safe.
- Side effects are mild and only last a day or two.
These vaccination fairs are only open to residents of the Humboldt Park and Garfield Park neighborhoods in Chicago, age 18 and up. If you need assistance scheduling an appointment please call us: (312) 432-4583
NOTE TO NON-HUMBOLDT/GARFIELD PARK RESIDENTS: If you are not from Humboldt Park or Garfield Park and use the information on this page to schedule a vaccination, you will be turned away. Once you schedule an appointment a member of the Erie team will check to make sure you are eligible based on the address you entered. Once Erie has confirmed that you are eligible, you will receive a text or email from Erie. You must have received this confirmation text or email in order to receive your vaccination.
APPOINTMENTS ARE REQUIRED
- No walk-ins
- No proof of insurance nor ID are needed
- No social security number is needed
You must be able to provide proof that you live in Humboldt Park or Garfield Park at the time of your appointment:
- Photo ID OR
- Utility bill
- Bank statement
- Other official mail with your name and address
If you do not live within the Humboldt Park or Garfield Park areas, you cannot receive the vaccine at these specific vaccine fairs.
What to expect on the day of your appointment

- COVID-19 screening

Please do not come to your appointment if:
- you have a fever or any flu-like symptoms
- you have tested positive for COVID-19 in the past 14 days
Not showing up for your appointment impacts the availability of the vaccine for other residents in Humboldt Park and Garfield Park.
